There are 2 ways to apply for AMFund financing - Online and Mail/Fax.
Both application methods will require the following documents:
A. Completed Application
B. One unbound paper copy OR an electronic file with your audited financial statements for
     the last three years.

Apply Online: This method will require the above documents in digital format for attachment to the final emailed application. In addition to the required files, an active email account on this computer will be neccessary for application submission. You may also want to print a copy for your records.

Apply by Mail/Fax: Print the completed application. The application form and the required documents may then be mailed or faxed to the following:
